What should I bring with me?
At the time of your first appointment, you should bring:
- Your Insurance Card
- Your Driver's License
- Completed Patient Information Intake Form (printable form is located at top of this page)
- Claim Number, if auto accident related (auto or workers compensation)
- Prescription from referring Doctor
- Any X-rays or MRI resulting pertaining to your injury
- Comfortable loose fitting clothing
When should I get there?
Please arrive at least 15 minutes prior to your scheduled appointment.
How long will I be there?
Expect to spend 60-90 minutes during your first visit. This time is to assure that each you are fully evaluated. The length of subsequent visits will depend on treatment and follow-up testing that will be recommended for you, but will usually be 30-60 minutes.
What will happen while I'm there?
During your visit, one of our physical therapists will discuss of your treatment needs with you.
First the Physical Therapist will review your history and conduct an examination. At this time, the Physical Therapist may make preliminary treatment or further diagnostic recommendations to you, and will also answer any questions you may have. |
